The following requirements must be met:

Ready to upload the second page of the Apprentice Application, completed and signed by your instructor
Ready to upload a Verification of your Instructor’s Qualifications, including 6,000 hours of his or her Licensed work experience
Ready to upload the Board approved Curriculum you will be using
Passing score for the Utah Massage Therapy Law and Rule examination
