Apply for a Licensed Advanced Substance Use Disorder Counselor License (Utah Certified Substance Use Disorder Counselor Applicants)

The following requirements must be met:

Have you completed 4,000 hours of experience while licensed as a Utah CASUDC?
NOTE: If you are a Utah SUDC and submitted at least 2,000 hours of experience with your SUDC application, you only need to document 2,000 additional hours
Supervisor(s) completed the Verification of Experience form?
