Exam Information

Exam Requirements Have Changed! (posted May 15, 2023)

Effective May 2023, the exam requirements for Indirect Supervision have changed. Please see the new requirements listed below. 


There are two classifications of licensure for Hearing Instruments: Hearing Instrument Specialist and Hearing Instrument Intern. The examination requirements for each are listed below.

Hearing Instrument Intern | Direct Supervision

  • Utah Hearing Instrument Specialists Law and Rule Examination *
    This examination is administered by the Division's exam vendor, Prov Inc. To register, please see the section below, "To Schedule an Exam with Prov".

Hearing Instrument Intern | Indirect Supervision

  • IHS Practical Examination for Hearing Instrument Specialists.
    This exam is administered by the Divisions exam vendor, Prov Inc. To register, please see the section below, "To Schedule an Exam with Prov".
  • IHS Distance Learning for Professionals Hearing Health Sciences Course.
    This course has an accompanying exam. For more information on this distance learning program, please visit the following site: IHS Distance Learning Course .

Hearing Instrument Specialist

  • Utah Hearing Instrument Specialists Law and Rule Examination *
    This examination is administered by the Division's exam vendor, Prov Inc. To register, please see the section below, "To Schedule an Exam with Prov".
    *Not required if you previously submitted a passing score as an Intern.
  • NBC-HIS National Competency Exam (NCE)
    You can request your examination application by submitting your inquiry to the following link: NCE Application Request

To Schedule the Law and Rule Exam:

  • Create an account at https://provexam.com/scheduler/ . You have the option to take this exam at most college and university testing centers in Utah or at home at home.
  • At-Home Testing: You also have the option of using an on-line proctoring test platform known as Examroom. To verify your computer equipment will run Examroom, please use this system check tool prior to registering for your exam: On-Line Proctoring System Check
  • Out-of-State Testing*: Contact Prov directly to make testing arrangements. The phone number is 877-228-3926. *This is only if you want to test in a live testing center.
  • For more information about this exam, please review the Examination Information Bulletins .
  • Electronic submission of exam results will be sent directly to DOPL from prov.
