The following requirements must be met:
- Supervisor review and signature on page 2 of the Apprentice Supervision Form
** ATTENTION INSTRUCTORS **
- The Apprentice Registration Application MUST be completed by the potential apprentice using their own name and account.
- Please complete the Apprentice Supervisor page and return it to the potential apprentice. The applicant is required to submit the completed Apprentice Supervisor page with their online or manual application.
