Apply for a Pharmacy Technician Trainee License

The following requirements must be met:

Training Program Enrollment.
If 18 or older on the date submitting this application, submit an original “Criminal History Report” from the Utah Bureau of Criminal History. For information on how to obtain the BCI report, please see bci.utah.gov . The report must be submitted as an original hard copy. Electronic copies of the report attached to your application are not acceptable.
