Nursing - APRN / CRNA FAQ's
Do I have to take the controlled substance examination?
Yes, if you want the controlled substance license. If you will be prescribing narcotics, schedule II-V, you must have a Utah controlled substance license and a DEA license. A CRNA who orders and/or administers a controlled substance must also have a controlled substance license.
How do I get my DEA license?
Contact the DEA for an application: (800) 326-6900.
How do I get an APRN intern license?
Submit the APRN application, the application fee, transcripts showing the Master's degree in nursing and the date the degree was conferred, the intern request form, and the intern license fee. The APRN intern license is issued after the degree has been earned, but before the certification examination has been taken.
Note: You are not eligible for a controlled substance license until you have been issued a full APRN license; therefore, you may not prescribe controlled substances under an intern license.
How do I apply for the certification examination?
You would need to contact the certification agency and find out their requirements.
You cashed my check, but why haven't I received my license or my authorization to sit for the exam?
It takes at least 10-14 working days to process an application. During April, May, or June it may take as long as 4 weeks to process a complete application. If you are applying by examination, and you have registered to take the exam, you will be made eligible at that time. You will receive the authorization to test 10 days from the date we made you eligible. If you are applying by endorsement, once the application is complete it will take another 10 days for the license to be printed and mailed.
I haven't received my authorization to test.
It takes at least 10-14 working days to process the application and up to another 10 days for Vue to send the authorization to sit. The application must be complete, including the complete application, appropriate fee, and transcripts showing the degree and date degree was conferred.
I've received my letter letting me know I have passed the exam. What is my license number so I can go to work? Can my employer call and get my number?
We print and mail licenses twice each week. You should receive the license approximately 5-7 working days after the effective date or date of the letter. Employers can check the web site at www.dopl.utah.gov.
I'm moving to Utah. How do I obtain a license?
Fill out an application completely and submit it with the current application fee and verification of your licensure in another state. A copy of the current license is not acceptable. Verification must come from the board of nursing.
I have never been licensed. Do I have to fill out the verification form in the application?
No. The application is used for both new graduates and for those licensed in another state. You only use the verification form if you have been licensed in another state.
Do I need to list all licenses and certifications I have been issued?
Yes.
I don't know the date I will take the exam. What should I put on the application?
We want to know the date you registered for the exam, not the date you will take the exam. This is the date you mailed the registration fee and registration form, or the date you called and registered over the phone.
What does "under interstate compact" mean?
An RN or LPN needs to be licensed in the party state in which he/she resides. The nurse may practice in any party state without obtaining any additional licenses. The nurse has one party state license from the home state, which is recognized by the other party states. Only when the nurse moves to another party state and declares that state as the primary state of residence, will he/she be required to apply for and obtain that state's license. This does not apply to non-party states.
The current party states are: Arizona, Arkansas, Colorado, Delaware, Idaho, Iowa, Kentucky, Maine, Maryland, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia and Wisconsin.
How long does it take to get licensed if I'm coming by endorsement?
If everything is complete, and there are no "yes" answers on the application, and everything is in order, it will take us 10-14 working days from receipt of a completed application. However, the application must be complete. If we are waiting for verification from another state, it is not a complete application and we have to wait for that verification.
